He eats dry food but awhile back he tried to drink some cream in a bowl and nearly did choke to death. He has no trouble drinking water and is healthy and playful. I am concerned it is getting a little worse. Any suggestions. I asked my vet about it last spring and she seemed unalarmed and said keep him on dry food.

For now, continue with the dry food and water and do not give the cat any cream - milk products can cause indigestion anyway. Watch the cat carefully whenever he is fed. Good luck! I hope you can find the answer soon.
I would take the cat to the vet again and ask specifically to have his teeth and gums examined closely. If no problem is found after that, then if you have a veterinary school within driving distance, I recommend you take the cat there, or to a vet who specializes in felines.
