I second what Martisimo has said! You probably need to see some sort of specialist or even take the cat to a veterinary college if there is one nearby. (Phone ahead for an appointment of course!)

Even the most clever vet can be stumped if a case is outside of his/her experience. It sounds like your cat has had the best of all possible care so far, but further avenues need to be pursued.
