If all is clear on the health front, you may want to consider re-training the cat to use the litterbox. This is hard work for you. He needs to be confined in a small room (like a bathroom) with the box, his bed and food. When he starts using the box, you gradually increase his territory until he has the run of the house again. PHCurious has already told you about deodorizing the places where the cat has peed. I also recommend a product called Feliway which is available at www.Feliway.com. This product mimics the happy scent used by cats when they rub their face against something. 80% of cats who have had urinary marking problems have stopped or noticebly decresased their marking. I have had tremendous success with this product myself. It is expensive, but you only use a little bit at a time. Finally, if all else fails an antianxiety medication often helps. My sister's vet prescribed Buspar for a cat with a marking problem and the problem disappeared. The medication was gradually withdrawn and since the cat had learned not to mark, the cat continued to use the litterbox successfully. The decision to prescribe this or any medication rests solely with your veterinarian who will need to give the correct dosage and monitor its use.
Please take the cat in to the vet for an exam ASAP. Three years is ample time to have developed a urinary infection, and if he does have one, it's going to harder to get him to start using the box.
