You may need to retrain the cat to the litterbox. If you can confine him to a small room with just a bed, food and the litterbox this might work. Then you can expand his area after he consistantly uses the litterbox. Talk to your vet about whether or not an antianxiety medication like Buspar might work. Very often calming the cat down will stop inappropriate elimination. Then the cat "relearns" that there is no need to mark and the meds can be gradually withdrawn.
The first thing to do is to take the cat back to the vet for a full evaluation. Since he was on antibiotics, this does indicate that there was probably a urinary infection invovled. Perhaps he needs another round to completely clear it up. Also the fact that his urine smells so bad leads me to believe that this might be a medical issue.
