It sounds like your cat has a case of litterbox avoidance. She may have associated any pain from pooping with the litterbox and is now going somewhere else.

You will probably need to retrain her too the box. Can you put her in a small room or large cage with just the litterbox, some bedding and food and water? After she learns to use the box, you could try letting her out and see what happens. Also if you do close her in a small room, surround all but a very small area with foil and she may be forced to use the box.

You might talk to the vet about prescribing some sort of antianxiety medication if this behavior persists. Since you are obviously treating her for a health problem, your vet may have a good picture of whether or not the bowel movement problems and the worms are affecting her choice to use the box.
